Job description - Care Team leader/care coordinator

We are looking forward for a Day Care Team Leader/care coordinator who can deliver the needs of the residents and the care home.

The post holder will usually be required to have a degree in the management or in some discipline within the health and social care. Day Care Team leader/care coordinator is responsible for providing leadership and supervision. To oversee the day to day management of the services. To actively participate and supervise in the delivery of care to the residents to ensure that their physical, social, intellectual and emotional needs are met and ensuring dignity, choice, independence and respect. To include appropriate liaison with residents, relatives, deputy/manager and relevant staff. Support and work closely with families and friends offering them professional and emotional support, sensitive to individual need. As a Care Team leader/care coordinator one will have to maintain, incorporate and improve the training plan for the care home. Ensuring the performance of the staff is of a high standard. Reporting to management. Discussing and resolving problems. Carrying out performance reviews of the care staff.

Job description - Health Services and Public Health Manager

• Implements policies of the board, ensures statutory procedures are followed, with particular emphasis on patient safety and the management of risk.

• Liaises with health care professionals to determine short and long-term needs and how to meet these objectives within budgetary constraints.

• Oversees the day-to-day management of the unit or service and provides leadership to staff.

• Uses statistical information to monitor performance and assist with planning.

• Negotiates and manages contracts with providers and purchasers of health care services; .

• Manages staff, including recruitment, appraisal and development.

• Monitors and reports upon the effectiveness of services with a view to improving the efficiency of health care provision.

Job description - Carer
Previous experience in EMI residential care work and NVQ 2 or 3 would be advantageous. Duties will include personal care, feeding, escorting to appointments and assisting with daily living. Job description - Staff Nurse
RNA or RGN Level 1 required. Should be NMC PIN holder or eligible to undertake nurse supervised practice/ONP. Responsible for the assessment of care needs and the development of programmes of care and/or implementation and evaluation of these programmes. To carry out and implement care plans; To liaise with GPs, Social Workers, Hospital Team and families; Administration of medication/drugs; Monitor patients' progress, apply surgical dressings and give other forms of treatment; Carry out routine investigations; Respond promptly to emergencies; Advise on nursing care, disease prevention, nutrition etc and liaise with the management on issues concerning nursing policy; Assist the matron to ensure that each resident receives care appropriate to individual needs; Assist the Manager to secure and sustain the home for the residents, promote a caring environment through high standards of professional practice conducive to the physical, emotional, social, intellectual and spiritual needs to the residents; Assist medical doctors, deal with psychological treatment of mentally ill patients; Monitor and supervise junior staff; Provide a high quality of care to the residents in the home.
